Here’s your chance to win $50 Gift from Amazon by filling in a simple, convenient survey form.
PATH and the Global Health Council are conducting a web-based survey on behalf of the Rockefeller Foundation, Results for Development, and International Health Policy Program of Thai Ministry of Public Health. The purpose of the survey is to assess the global health community’s attitudes on financing and provision of health services in low- and middle-income countries.
The survey is being conducted online and should take about 20 minutes to complete. If you have experience with low- and middle-income countries and are willing to anonymously share your perspective, please go to the following Internet address to complete the online survey:
We would greatly appreciate your response as soon as possible as the survey will officially close on June 27, 2008. Your perspective is critical to the success of this project. We hope that you enjoy the opportunity to provide your valuable opinions.
Your individual responses will be kept strictly confidential and the information you provide will not be used for any other purpose. The survey is anonymous and we will not collect personal identifiers, including your IP address.
If you have any questions or need the survey in another format, contact Dai Hozumi, the lead researcher of this survey, by sending an email to dhozumi@path.org.
As a token of appreciation for your participation, we will randomly select 10 respondents to receive a US$50 amazon.com gift card. If you want to be included in the drawing, please enter your email address at the end of the survey. Only one entry per participant is allowed and email addresses will only be used for the purpose of the drawing. Winners will be notified by email on July 15, 2008.
Thank you in advance for your participation.
